Preschool Forms: All students registered in Bright Beginnings preschool must have the following forms on file before attending class: Emergency Form, Medical Authorization & Release of Liability Form, Physician's Report (Child's Immunization Form), Student Agreement - 1 Day Classes, Student Agreement - Multi Day Classes, and a valid TB test for the attending adult. Return completed forms to the Parent Ed office, Room 4 at the Adult Ed Center before attending your first class.
